When going on outdoor activities, you should consider carrying a list of emergency contacts and any other information that might be relevant in an emergency.

The form, below, is intended as a guide only. Use it at your discretion but you should include whatever emergency response and contact information that is relevant to you, both current and should your circumstances change.

You can download the form, or just use it as a checklist to help ensure everything relevant to you is included.

Carry the information in a plastic bag in your pack where it can be easily located.
